Hoyleton’s Wraparound program serves children and youth ages 0–21 with complex emotional, behavioral, or mental health needs—especially those with open DCFS cases.

What Is High-Fidelity Wraparound?

High-Fidelity Wraparound (HFW) is a trauma-informed, family-centered process for youth (0–21) with complex needs, especially in child welfare.

Objectives: stabilize families, build lasting supports, and promote family voice.
Components: family-driven planning, team coordination, blended supports, and outcome tracking.

Eligibility Criteria for HYFS Wraparound

Youth must meet the following to qualify for Wraparound:

Open Intact or Permanency case with DCFS
Lives in or has case in Southern Region (4A): St. Clair or Madison counties
Ages 0–21
Complex behavioral, emotional, or mental health needs per CANS 2.0
Exceptions approved by DCFS Administrator

How to Refer a Youth

Referral Submission: Submit Referral Form, CFS 600-3, Integrated Assessment, and latest CANS Assessment to wraparound@hoyleton.org
Timeline for Initial Contact: HYFS contacts the referring caseworker within 48 hours. Wrap Facilitator contacts family and case manager within 48 hours of assignment.
Eligibility and Initial Meeting: Complete/update CANS within 30 days, then hold the first CFTM and finish Intact enrollment forms.

The Wraparound Process

WEEKS 1-2

Engagement

Build rapport, explain the Wraparound process, and identify strengths and needs.

WEEKS 2 - 4

Plan Development

Co-create an Individualized Plan of Care with the youth, family, and support team.

WEEKS 5 - 16

Plan Rollout

Carry out the plan; the Wrap Coordinator facilitates monthly Child and Family Team Meetings.

WEEKS 16 - 36

Transition

Gradual step-down in services, creation of a sustainability plan, and formal case closure

Roles & Collaboration

HYFS Wraparound Coordinator: Facilitates the process, runs CFTMs, and ensures fidelity to the 10 Wraparound principles. Acts as the bridge between the family, DCFS, and providers.
DCFS Caseworker: Shares case details, participates actively in plan development, and partners throughout the process.
Family and Natural Supports: Lead decision-making, identify goals and supports, and help guide the plan to ensure it reflects their strengths and needs.

What are wraparound services?

Wraparound programs use a strengths-based, needs driven approach to help youth and families reach positive objectives and enhance their overall well-being.

What is Family voice and Choice?

Families are actively involved in all aspects of the wraparound process, from planning to implementation and evaluation. Their perspectives, needs, and preferences are prioritized.

Who does Hoyleton work with?

Illinois residents that have Medicaid or clients that qualify for services through specific grant funding. Assessments must be done to determine what services can be provided. Clients are also referred to Hoyleton through state agencies, most commonly Dept of Children and Family Services (DCFS), Illinois Depart of Healthcare and Family Serv ices (HFS), Illinois Department of Human Services (IDHS) and the Illinois Department of Public Health (IDPH).

What is a CFTM?

It stands for Child and Family Team Meeting—a gathering of families in the child welfare system and their supports to create a plan based on family strengths and goals. The meetings also address needs identified in the child protective services assessment.

What are Community-Based services?

Services and supports provided in the least restrictive and most inclusive settings within the family's community.
