Supporting Youth in Transition to Independent Adulthood


Hoyleton’s Transitional Living Program (TLP) provides housing and life-skills support for youth ages 17–20 with developmental delays exiting foster care. In supervised homes, they learn finances, self-care, and employment skills while building confidence, healthy relationships, and independence through community resources.
The small number of clients allows staff to build strong relationships and provide care tailored to each youth’s developmental level. Every youth has a customized plan outlining goals for independence, including healthcare, employment, education, and transportation. They also practice socialization and problem-solving skills. With consistent, trauma-informed support, youth gain confidence managing real-world responsibilities. The program creates a stable foundation where young people feel heard, supported, and empowered to move forward.
