Wraparound Program

Wraparound serves individuals and families with open Illinois Department of Child and Family Services (DCFS) cases in Madison and St. Clair County
High Fidelity Wraparound provides a comprehensive and holistic method of supporting individuals, within the child welfare system, who are experiencing complex behavioral, emotional, or mental health challenges. This program employs a family centered, trauma informed, strengths based process that brings together the youth, their family, their Case Worker, as well as other service providers that the family identifies. Together the team creates a plan that capitalizes on the individual and collective strengths of the family, setting goals to overcome obstacles and address underlying needs.

Eligibility Criteria

- Open Child or Family Case with DCFS Intact or Permanency
- Case in the Wraparound Target Area (See map to the left)
             - Child/Family living in the Target Area
or 
             - Legal case in the Target Area
- Youth ages 0-21 with complex behavioral, emotional or mental health needs of the youth as defined by specific CANS 2.0 items
- Exceptions may be made by the DCFS Wraparound Administrator

Referral Process

- Referral to HYFS - 48 hours to contact caseworker
- Once assigned Wrap Facilitator has 48 hours to contact family & case manager
- Staff with case manager to complete CANS for eligibility with 30 days
- If eligible meet with case manager and family for the initial CFTM 
- Complete Intact forms for enrollments
- Wraparound Coordinator facilitates CFTM's every 30 days for process

What Documents Do I Need To Participate?

- Referral Form
- Release of information/ROI (CFS 600-3)
- CANS from case manager
- Send email to counselingreferral@hoyleton.org

Wraparound Process

- Engagement (Weeks 1-2)
- Plan Development (Weeks 2-4)
- Plan Implementation (Weeks 5-16)
- Transition (Weeks 16-36)
